Our post-acute care facility offers comprehensive rehabilitation services and 24-hour skilled nursing and supportive care designed to meet the unique needs of those we serve for best possible recovery. With industry leading 1:3 staff-to-patient ratio, 1:1 therapy and 24/7 care, Sunrise offers individualized care plans with our Interdisciplinary Treatment Team composed of physicians; nurses; physical, occupational, speech and respiratory therapists; dietitians; case managers; social workers, neuropsychological specialists; and activities directors.

Sunrise Congregate Living Health Facility is located in the heart of Los Angeles, city of Van Nuys. We are a 6 bed facility with 6 private patient rooms newly renovated and equipped with the state-of-the-art technology and medical equipment. The 2,500 square-foot facility also features a spacious living room and dining room as well asa big outdoor area with green backyard to make our patients feel they are at home.
