The primary need of congregate living health facility residents shall be for availability of skilled nursing care on a recurring, intermittent, extended, or continuous basis. This care is generally less intense than that provided in general acute care hospitals but more intense than that provided in skilled nursing facilities.

Our specialized team of Registered Nurses (RN), Licensed Vocational Nurses (LVN), Certified Nursing Assistants (CNA), Dietitians, Speech Therapists and other professionals offer personalized person-centered care to our residents ensuring high quality medical care in a comfortable home like environment.

Here at Sunrise Congregate Living Health Facility we believe that high quality medical care starts with person-centered approach where each patient is put in the center of his/her healthcare. We encourage our residents and their families to participate in making their care plan. Each resident's comprehensive care plan is designed to incorporate and identify problem areas, incorporate risk factors associated with the identified problems, build on the resident's strengths, reflects resident's expressed wishes regarding care and treatment goals, reflect treatment goals, timetables and objectives in measurable outcomes.
